What is the “effective cost per month”?

When you’ve got hundreds of different tariffs with a varying upfront costs and monthly line rental charges it can be difficult to work tariff works out as being the best value.     18th June 2011: This article has been updated due to O2 price increases on the Galaxy S II. The best value mainstream tariff is now Three/T-Mobile.
